電腦化溝通教學介入：The SCOPE Trial, CD-ROM training program

■ 研究設計：the SCOPE (Studying Communication in Oncologist Patient Encounters) trial

可區分為三階段：一、收案、前測與1小時溝通技巧課程；二、CD-ROM（唯讀光碟）介入；三、後測。第一階段中，研究團隊招募醫師與病人後，對看診/會談過程進行錄音，以及資料收集；錄音會作為後續介入的材料。不論介入或控制組，所有醫師皆接受1小時溝通技巧影音課程。第二階段中，研究團隊利用前測之錄音，製作訓練光碟，並寄送給介入組醫師，有一個月時間觀看。第三階段所有醫師再進行其他病人看診/會談之錄音，並進行資料收集。

■ 介入方法：tailored CD-ROM

首先，研究團隊製作CD-ROM訓練之內容，並從第一階段各醫師完成的看診/會談錄音中擷取範例。當目標行為在會談過程中出現時，研究團隊使用自己開發的軟體，對錄音資料進行編碼/標記。接著製作客製化版本之CD-ROM，搜尋並夾帶該位醫師的關鍵錄音片段，研究團隊亦會寫下一些回饋。

CD-ROM訓練課程共有6個模組，前5個條列如下，最後一個為總結與複習：

1. principles of effective communication

2. recognizing empathic opportunities

技巧包含：direct patient emotion、indirect patient emotion

3. responding to empathic opportunities

技巧包含：addressing emotion、responding to emotion、distancing and blocking、emotion-handling skills

4. conveying prognosis

5. answering difficult questions

每個模組約10~15分鐘，內容包含：(1) 介紹技巧；(2) 1~2個示範影片；(3) 總結重點；(4)查看來自自己錄音的例子與個別化回饋，瞭解溝通技巧使用情形。

4. 參加者進行醫病會談時錄音，以在後續獲得個別化溝通技巧回饋
